Consent to Treat a minor. We treat people of all ages but for patients younger than 18, we will need a legal guardian to fill out the below form for consent

We want to give you the best and most comprehensive care possible. In some situations, we may need to request medical records and will need authorization to do so. We want make sure we understand all aspects of your health in order to provide you with the most effective care. Often times, we will request this form be filled out after speaking with you or your initial consultation.
